Goldenrod
The Goldenrod Form is used to certify that a student has successfully completed all requirements for the Psychology degree. Students should complete the Goldenrod Form one semester prior to the semester in which they plan to graduate. Goldenrod Forms are available from the Colleges of Arts and Sciences (QLC 113), the Psychology Department Office (Gartley 110), and the Psychology Undergraduate Advising Office (Gartley 14). You can also download the goldenrod form. Students should complete Side A of the Goldenrod Form and submit the form to the UAO. You can also email a copy to psychadv@hawaii.edu. A Psychology advisor will verify the student’s completed courses within three to five days and will notify the student when the Goldenrod is ready for pick-up. Completion of the Goldenrod Form allows students to sign up for a mandatory GRAD Session through the Colleges of Arts and Sciences, to verify that they have met the core general education requirements for the Bachelor's degree.
Important Instructions
- Type or write in ink (do not use pencil)
- Record ALL psychology courses that appear on your transcripts, including those that do not count towards your major requirements (courses with a W, grades C- and below, and duplicate courses).
- Do not record courses in which you are currently enrolled.
- Only fill out Side A
